• Other geriatric case management experience
• At least 2-5 years of experience working with older adults or adults with a need for ongoing case management, chronic or progressive illnesses and the impact on families/caregivers.
• Must have some knowledge of the elder continuum of care.
• Must be comfortable working within and learning new software and technology tools that support ageing.

• Shift Start/End Time: Days 8-5
• Dress Code: Professional

Job Description: – Provides social work assistance to patients and families as related to social determinants of health.

Specific Duties:

• Interviews patients, assesses priorities and documents case activity.
• May consult with multi-disciplinary team members in liaison with community resources to support the patient’s treatment.
