Job Description.

The Social worker’s role will be to assess and work with people to ensure care and support plans are put in place, building on the person’s strengths and assets along with formal support if needed. This role provides the opportunity to work collaboratively across health and social care to meet individual needs holistically. The service is proud of its effective partnership working with various health colleagues.

Responsibilities.

  • Carrying out strengths-based and person-centered assessments as outlined under the Care Act 2014 for individuals and their carers.
  • Applying legislation relevant to the role, including Care Act 2014, Mental Capacity Act 2005, etc.
  • Working together with professionals across health and social care to meet individual needs.
  • Manage a caseload with support from the line manager.
  • Manage to compete for demands to maintain flow through the discharge services.
  • Complete safeguarding inquiries, mental capacity assessments, and best interest decisions with guidance from the line manager.

Requirements.

  • Someone with a Social work qualification (DipSW, CQSW, Degree in Social Work or equivalent).
  • Someone who is registered with Social Work England
  • Someone who understands safeguarding adults and is working towards or has the safeguarding inquiry.
  • Someone who can drive (or have a driver) and has access to a motor vehicle.
  • Someone with good verbal and written communication skills
  • Someone able to make decisions and demonstrate sound risk analysis.
  • Someone able to work as part of a team and with other professionals and agencies
  • Someone willing to be flexible and want to develop as a professional and empower others.
